Mental Health Intake Information

1. For all psychological testing and therapy inquiries, please call 616.244.2246. Our receptionist in Jenison will take down your child’s demographics, reasons for seeking services, and insurance information to get you started with the intake process.

 

2. DE will then call your insurance company(s) to check your Mental Health benefits. Once we know what type of coverage you have, we will give you call and relay that information to you.
PLEASE NOTE: Checking your insurance benefits is a courtesy DE provides. We recommend that you also call to verify your insurance coverage, as this is not a guarantee of payment.

 

3. Before calling you regarding your insurance, we will also have our Clinical Director take a look at your intake information and determine which one of our clinicians will best fit your child’s needs. Our receptionist with then call to go over insurance benefits, let you know which clinician your child was assigned to, and offer the opportunity to schedule the first initial appointment. Note: If we are unable to schedule at that time due to a wait list or lack of availability, we will give you an estimated time frame of how long it will be until a clinician will be available.

 

4. For children and adolescents, the first appointment we schedule will be a parent-only appointment. This initial intake is for parents only (at least one parent present) and will allow the clinician and parents to discuss all things regarding the child and the reasons they are seeking services, without having to do so in front of the child.

 

5. Once the initial appointment is complete, the clinician will either set up a time for the testing to take place, or discuss options for reoccurring therapy appointments. If you have any questions regarding your schedule, please feel free to contact either our Holland or Jenison receptionists.

ABA Intake Information

1.To inquire about psychological testing for autism, please call 616.244.2246. Our receptionist in Jenison will take down your child’s demographics, reasons for seeking services, and insurance information to get you started with the intake process. If your child has been tested elsewhere, please call 616.604.8492, and our receptionist in Wyoming will assist you with the intake process. Please do not complete forms until instructed to do so by receptionist.

 

2.DE will call your insurance company to check for ABA coverage. If you are part of a Medicaid plan, you will need to go through your local county Community Mental Health to obtain ABA coverage and authorization for treatment. PLEASE NOTE: Checking your insurance benefits is a courtesy DE provides. We recommend that you also call to verify your insurance coverage, as this is not a guarantee of payment.

 

3.After testing and obtaining authorization for services, a receptionist will reach out to complete intake questions. At this point the information will be forwarded to our Director of ABA to review and determine which one of our Board Certified Behavior Analysts will best fit your child’s needs. The BCBA will schedule an initial assessment with your child, followed by a parent meeting to discuss scheduling and findings from the assessment. Note: If we are unable to schedule with a BCBA due to a wait list or lack of availability, we will add your name to the waitlist and give you an estimated time frame of how long it will be until a clinician will be available. Please keep in mind that this estimate is not a guarantee.
